Hospital-based outpatient program for older adults who have concerns about their physical health or memory * provides referrals to Baycrest programs and services as needed * services provided by geriatricians and other healthcare professionals

Geriatricians will be able to:

assess your physical and mental health
recommend a personalized treatment plan
refer you to the appropriate specialty clinics, programs and support services at Baycrest or elsewhere, as needed
provide recommendations to your family doctor to assist in coordinating your care plan
