Physician/Nurse Practitioner referral is required, though clients without family doctors can access referrals through walk in clinics

Admissibilité / population desservie

Seniors 65 years and older or physiologically and/or cognitively equivalent who have:

deteriorating health or functional status
safety concerns
frequent or increased use of health services
altered quality of life

Residency Requirements: For seniors living throughout Northwestern Ontario (Clients living in the city of Thunder Bay can access Geriatric services at St Joseph's Hospital)

Provides a comprehensive geriatric assessment via telemedicine (using videoconferencing and other electronic equipment) with a focus on identifying resources and strengths of the client

Community services are identified and/or initiated to keep clients in their home and community and a coordinated care plan is delivered based on the assessment needs of the client and their families
