Hospital-based outpatient rehabilitation for adults who have had a stroke, acquired brain injury, spinal cord injury, or have complex musculoskeletal conditions * provides assessment and time-limited treatment to:
maximize recovery from a recent acute event
maximize functional abilities and patient safety
promote adaption of/to home
support transition to appropriate community-based programing
support timely transition from acute care to inpatient rehab setting
prevent admission or re-admission to an acute or rehab hospital
provide opportunity to learn/practice in a familiar, stimulating, and supportive environment
provide goal-driven treatment based on measurable progress and functional change
