Healthy Smiles Ontario

Children must be:

17 years of age and younger
a resident of Ontario and
a member of a household with an Adjusted Family Net Income (AFNI) at or under the levels shown here

Non-Insured Health Benefits

Required to present status card

Ontario Disability Support (ODSP)

Required to present dental card issued by ODSP

Ontario Works (OW)

Requires dental card issued by the Ontario Works' office; proof of eligibility for the month must be shown
