Utilizes an outreach, community-based case management approach to help clients with severe and persistent mental illness and clients with mental illness and co-occurring substance use disorder overcome problems related to problematic alcohol/drug use or dependence and stabilize their mental illness
Develops an individualized treatment plan that can include:
- Counselling
- Psychiatric assessment and follow-up care
- Pharmacotherapy
- Group therapy
- Psychoeducation
- Therapeutic recreation
