Referrals can be made through the Thunder Bay Mental Health Programs Intake Coordinator or a completed Thunder Bay Mental Health Program Referral Form can be submitted through the Access Point Northwest website
Referrals are accepted from clients, physicians, and community professionals and agencies
Some services require referral directly from a physician or nurse practitioner
