Family and community health care team* coordinates patient care * team includes family doctors, nurses, nurse practitioners, dietitians, social workers, occupational therapist, diabetes educator, and foot care nurse * after hours care available
Virtual Care
Appointments with a specialist or health care provider using a two-way secure videoconference technology * allows patients to access a specialist anywhere in Ontario * central site for coordinating the appointments for residents of Kingston, Frontenac, Lennox and Addington counties
