Description of Services

Nutrition, transportation, in-home and wellness services to support the health and well-being of people in Northumberland County 
  •  
  • Friendly Visiting Volunteer provided on a one to one basis to visit on a regular basis * visit activities may be playing cards, walks, crafts, conversation * volunteers are screened * can provide service for persons with dementia
  •  
  • Home At Last - Hospital Transition Program Transitional supports when being discharged from hospitals in Northumberland County
  •  
  • Home Help and Maintenance Program Assistance with light cleaning * shopping * meal preparation
  •  
  • Hospice Palliative Care Program - Visiting Hospice Service Trained volunteers provide emotional, psychological, spiritual and social supports
  •  
  • Meals On Wheels Nutritious hot or frozen meals are delivered during the noon hour by volunteers to the person's home * special diets can be accommodated
  •  
  • Personal Distress Alarms Medical alert system * allows older adults and people with disabilities to call for assistance using a help button * 24-hour emergency response
  •  
  • Specialized Transportation - Accessible and Rural Services Transportation to medical appointments, shopping and social activities and programs
  •  
  • Support and Care for Caregivers Assistance with the on-going responsibilities of caregivers who are caring for a loved one, friend or neighbor * peer support and discussion for caregivers and family and friends to share coping strategies and information on resources
  •  
  • Telephone Reassurance Calls Regular telephone contact provided in order to provide reassurance and arrange for assistance if necessary * normally short five minute calls at regular, prearranged times or when family and friends are away
