Patient must be assessed and accepted by the program director, unit manager, or other specially trained assessors

Eligibility / Target Population

Depending on bed availability, patients can be accepted to a bed at the Chatham Site

Fees

Daily fee known as "Chronic Care Co-payment" set by the provincial government may be charged while a patient is on one of these units * Fee Reduction: Dependent on financial information provided for co-payment

Description of Services

Chatham-Kent Health Alliance’s (CKHA) Complex Continuing Care (CCC) Unit provides specialized inpatient care for medically complex patients, offering both short-term, goal-oriented care for those with potential for improvement, and long-term support for individuals with chronic conditions. The unit focuses on stabilizing patients and maximizing their functional abilities, with tailored therapy and medical oversight. For those with restorative potential, the aim is to facilitate a safe transition to appropriate community settings like assisted living or rest homes. For patients with long-term needs, the unit provides ongoing medical and nursing support. The CCC unit also serves as a transitional space for patients awaiting placement in alternate living arrangements or for low-intensity, “slow-stream” rehabilitation care for patients unable to tolerate high-intensity rehabilitation directly from acute care.
