Description of Services
Support to seniors and their caregivers who reside in the community * promotes independence, education, and healthy aging * helps prevent hospitalizations with in-house respite services to seniors, also develops care plans based on clients' needs and preferences, and connects clients with resources * maximizes the ability of high-risk seniors to live independently in their homes and prevent early admission to long-term care facilities
