Provides supports to safe and prompt care for matters not requiring treatment at a hospital * aims to ease pressure on long term care waitlists to allow people to remain in their own homes * supports hospital discharge for long term care bound patients * Offered 24hours everyday, to residents who are:

• on a long term care waitlist
• soon to be on the long term care waitlist
• opted to remain in their own homes
• patients requiring supplemental Community Paramedicine services, beyond home and community care
• patients who may otherwise benefit from community paramedic involvement
