Description of Services

Nurse Practitioner-led multidisciplinary clinic providing safe, accessible and gender affirming care within a multidisciplinary, shared care model * team includes a Registered Nurse (RN), Registered Practical Nurse (RPN), Social Worker, System Navigator, and Pharmacist

Services include:

Support for social, medical and legal transition processes
Education and information on gender identity and healthcare options
Surgical assessment, referrals and support
Sexual health education and screening
Individualized support for hormone therapy and monitoring
Psychosocial support throughout the transition process
Group therapy sessions and workshops
