Adults 60 years and older who are being discharged from hospital, must be medically stable and able to transfer independently

Description of Services

Free community support services to promote successful discharges from either in-patient hospital stays or emergency departments * provides individuals to access services for a 4-6 week period to regain strength and independence * services may include:
  • up to 15 frozen meals
  • transportation home from hospital
  • transportation for 2 follow up medical appointments
  • homemaking services
  • home safety assessment
  • communication with other support services
