Eligibility / Target Population

ALC or potential for Alternate Level of Care (ALC) in acute care who requires complex care needs upon discharge in the community; preferably those with active Long Term Care(LTC) placement application or waiting for crisis placement
Generally frail with multiple comorbidities
Patient needing behavioral support; must have a geriatric assessment

Description of Services

Care for patients discharged from hospital/RCC that are deemed Alternative Level of Care (ALC) patients

Criteria for Program:

55 years and above, recently discharged from the hospital deemed ALC
LTC application completed prior to admission to CANES Transition Care beds (length of stay is 42 days)
For clients with the community as the post discharge plan, client and family discussion is in place before the admission from the program

Southbrook Retirement Community (400 Ray Lawson Blvd, Brampton, ON L6Y 4G4)
Sorrento Retirement Residence (10 Station Rd, Bolton, ON L7E 4L3)
