Description of Services

Programs and services provided by an interdisciplinary team of health professionals that focus on prevention and wellness, information and health promotion, as well as primary care

  • Chinese Cancer Support Group Support group for cancer patients and their family members in accessing cancer care services.
  •  
  • Dental Bus Dental services delivered through a partnership between SRCHC and the Toronto Public Health Mobile Dental Clinic * run by a dentist and dental hygienist * appointments are necessary
  •  
  • Midwifery and Toronto Community Health - MATCH Prenatal, labour, birth and postpartum care with a team of midwives * services offered at Michael Garron Hospital, Toronto Birth Centre, or at home * 24h on-call services to MATCH clients during pregnancy and for the first 6 weeks postpartum * home visits to provide clinical care in the first week postpartum * gynecological care (Pap tests), STI screening, pregnancy testing (urine or blood tests) and counselling related to pregnancy options * support and teaching on testosterone injections

  • Outreach Clinic A drop-in service that facilitates access to ongoing health care for medically unattached individuals who meet a specific criteria
  •  
  • Primary Care Asthma Program (PCAP) A certified respiratory educator provides education and care for children and adults living with asthma or chronic obstructive pulmonary disease (COPD)

  • Social Work Support Counselling, case management and advocacy, including housing, income, immigration, family and relationship issues, feelings of depression and anxiety, coping with illness, dealing with past or current problematic drug and alcohol use

About Us

Our goal is to help clients take control of their own health as much as possible, as well as help them access the programs and services they need. Clients are helped by our interdisciplinary team that includes doctors, nurses, nurse practitioners, dietitians, chiropodists, physiotherapist, social workers, health promoters, educators, case workers and harm reduction workers.
