Description of Services

Self-management, prevention and education rehabilitation program for patients and family members who have been diagnosed with a coronary artery disease * cardiovascular rehabilitation team includes cardiac rehabilitation therapists, clinical pharmacist, medical doctor, psychiatrist and registered dietitian

Services offered include:

medical evaluation
nutritional counselling
cholesterol management
pharmacy counselling
heart health education
stress management
spouse and family support
support to stop smoking
personalized exercise assessment
