Description of Services

Non-emergency supports for older adults who are experiencing mental health issues along with age-related challenges * services include:
  • mental status assessment
  • medication review
  • counselling
  • recommendations for treatment options
  • education and follow-up for clients, family members, and caregivers
  • connection to community services/resources
  • advocacy with other community support services and family members
  • home visits
