Description of Services

Teams provide comprehensive integrated services to adults living with a severe and persistent mental illness

Services include:

assessment
individualized treatment plans
coordination of care and services for clients
one-on-one counselling and support
life skills training
monitoring of the mental state of clients and their use of medications
vocational and recreational activities and programs
family education and support
referral and advocacy services to community agencies
