Services for people and families facing life-limiting illness, death and bereavement * in-home hospice palliative care support throughout Scarborough

Two programs with various opportunities:

Hospice Palliative Care - one-on-one in-home visits, Day Hospice, Caregiver support group
Bereavement Care - one-on-one visits, various support groups
