Care and support for children and families living with life-threatening or life-limiting illness or disease * multidisciplinary team of physicians, nurses, social workers, child life therapists and spiritual advisors

Services include:

pain and symptom assessment and management
written Advance Directives
connections to community palliative care agencies and services
psychosocial support and written resources for patients, parents and siblings
bereavement care
assistance with creation of memories and legacy
