Provides enhanced personal support care services to seniors that require additional personal care services in order to remain safely in their own homes
Can be used independently or in addition to other professional services offered by Ontario Health atHome
Each Care plan is specifically designed to fit the needs of each individual and may provide:

• 24/7 access to care
• daily phone support or check-ins
• assistance with activities for daily living
• light housekeeping duties
• meal preparation
• assistance with banking, paying bills
• recreational therapy
• socialization
• respite for caregiver
• palliative care support

Packages are categorized into Low, Medium and High Maintenance Care Plans, tailored to suit individual needs.
Assisted Living Program will provide 70% cost of care; 30% cost is provided by the client
