Eligibility / Target Population

Individuals who are 18 years of age or older, and
Have a valid Ontario Health Card
Have health care needs that cannot be met with any combination of caregiving, care in the home or care in the community
Have health care needs that can be met in a long term care home

Fees

Rates set by the Ontario Ministry of Long-Term Care * subsidies for basic rates may be available for eligible applicants * some programs/services may require additional fees

Description of Services

Operates a 544 bed long-term care facility offering a variety of programs, services and amenities

Offers 17 Resident Home Areas with 32 residents each

Provides a dining area, living room/lounge area, activity room, and spa room within each Resident Home Area

Coordinates recreation and leisure activities for residents, including:

Bingo
Community Outings
Exercise Groups
Music Therapy
Pet Visitation/Therapy
Socials

Provides residents with the following services:

24-hour Nursing Care and Supervision
24-hour On Call Physician
Care Conferences
Foot Care
Hairdressing/Barber Services
Laundry Services
Meals
Medication administration
Palliative care (end-of-life care)
Physician Visits
Social Work Services/Family Services Coordinator
Spiritual Programs and Pastoral Care
