Primary care providers (family doctor or nurse practitioner) or community endocrinologists can refer a patient by contacting the Mississauga Halton Central Intake Program * adults interested in attending a Diabetes Education Program can also contact the Mississauga Halton Central Intake Program at 905-338-2983 ext 4872 to initiate the referral process (self-referral)
Eligibility / Target Population
Adults 18 years and older with pre-diabetes, type 1 diabetes and type 2 diabetes * paediatric (5-18 years old, seeing only at Oakville location)
