Eligibility / Target Population

Older adults and people with dementia * care coordinators assess the needs of applicants

Fees

$15 per day * $15 additional fee for transportation * $35 per night for overnight respite

Description of Services

Social and recreational supports in a group setting for older adults with Alzheimer's disease or related dementia, age related vulnerabilities, and/or chronic illness * opportunities for social, spiritual, recreational, and therapeutic activities are facilitated * provides a break for family/care partners who are responsible for day-to-day care
  • recreation/ leisure activities
  • intergenerational programming with other on-site programs
  • special events and entertainment
  • pet partnership
  • daily exercises
  • spiritual care programs
  • meals and snacks provided

Overnight respite offered Friday to Sunday for older adults currently being supported by a care partner or family member and enrolled in the Adult Day Program * staff provide 24-hour care throughout the weekend based on individual need

Companions in CARE - 6-week dementia care partner support and education series offered throughout the year
